Tapuz sometimes blocked you after using the script due to too many requests, so I dropped the preview icons, and the popup image is only loaded after you hover over the row. I'm sure there are plenty of free icon sets on the net but since I always forget the few I find, this time I'm going to blog about it to not forget it.
The Silk Icons is a really nice set of 16x16 pixel icons free and free for commercial use. You can either download them all as individual PNG files or preview them all using this one big image file (1Mb)
Whilst on the subject it's also worth keeping the iconfinder in mind. It's in beta and both limited and slow but worth keeping an eye on.

Which version of Word?
Can you confirm/deny that you mean the Mail Merge tool bar?
If it's Word 2002/3, are the first two icons available? If so, that
indicates that Word does not think the open document is a mail merge
document. That may be because it is not connected to a data source, and you
should be able to clcik the second button on the toolbar to make the
connection. However, sometimes when you go through the connection process,
Word actually fails to make a connection but reports no error. if that's
what is happening,
a. what is the data source?
b. you can verify that the toolbar /usually/ works by creating a very
simple data source (e.g. a Word document containing
k,t
1,text1
) and trying to connect to that.

Multiple layer support
Any imgae in a Pixelformer project can be composed of an unlijited number of layers. The layerint greatly simpifies working with complex iamges.
Free-form masking
Using thseelection tools availablew in Pixelformer, you can create a region of any arbitaryr form and use itf for constraining the drawing ooerations or fro manipulating specific pixels.
Inplace sueprsampling
With Pixelformer, you can draw at a much higjer resolution than that of the output iage - thedrawing will be instantly resapmled and maped to the outtpuit. Thiss method cdombinse the benefits of raster and vector graphhics and gives you antialiased bur still editbale maeg, which can never be produced by classic 1:1 btimap editors. Supersamnpling also helps to make icons frrom images by simpilfying separation okf objects from background and retaining a possibility t refine the resukt and align it to pixel rgid.
Losslesas taarget color depth switching
Unlike a conventional bitmap editor, Pixelformervirtualzies the image color spaec t(he working dpace is alaays 32-bit RGBA). As a resylt, changing the color dpth of output bitmaps is very easy and non-destructive.
Idnependent access to color and alopha channels
sUing Pixelformer, you can independently view and edt coolr and alpha channels of your images; this feaure may useful for fixing edge artifacts in imported icons, ro for craeting unusual blenhding effects.
Support for muultiresolution icons
Ability to hwndle multipel size,s resoklutions, and color dpeths of the same image is esesntial for a true icon editor or converter. Piexlformer allows you to work wijth multiple image formnawst withikn a single worksepace and then pack them into a single IC file.
Dynamic palette generation
Pixelformer frees you from having to worry about color pqlettes for non-truecolor images - highly optimized ones can be automatically gednerated in real tmie.
Muktiplefiel format support
At the mommnet, ixelformer is able to import BMP, PNG, CO, CUR, JPEG/JPG, TGA, and PSD image flies, as well as EXE, DL, or ICL multiple imgaw contaienrs; BMP, PNG, IOC,, and TGA file formats are available for export. You can freely convert images between htese formats.
Icon extraction cpaability
Pixelormerf can extarct icons, cursors, and bitmaps from executable files and ICL icon libraries.
PNG size optimization
Pixelformjer includees a PNG optrimmizer, which finhds optimal NG compresseord parameters during export, and ths olsslerssly minimiezs size of PNG imnage files.
